A Quick Look into the MRCPCH Clinical Exam Dates- October Diet

Know your stations

As you already know, there are 10 stations in the MRCPCH Clinical Exam. Each of these stations will assess your ability to communicate effectively, take a focused clinical history and carry out a clinical examination. The stations will assess your knowledge of expected developmental milestones, and may assess your abilities in counseling. Some stations may assess your knowledge of ethics. Other stations may assess your ability to analyze video clips and/or auditory clues.

The stations are usually going to cover:

• Communication – breaking news, counselling, or navigating a difficult conversation with a parent or carer
• History Taking – focused, structured history-gathering against the clock
• Child Development – assessing a child against expected developmental milestones (often run across two variations)
• Video stations – interpreting recorded clinical scenarios
• Clinical examination stations – typically covering abdominal, respiratory, cardiovascular, musculoskeletal, and neurological systems

Each station is scored against explicit domain-based marking criteria. Notably, some candidates fail to achieve a mark for clinical examination even while they may have had adequate clinical knowledge for the case being presented.  Knowing the schemes and criteria is going to aid you tremendously. Only then will you be able to identify what you are going to be assessed on and what will determine your final score.

Practicing transitions is as important as knowing the cases

Candidates usually feel prepared for the station content and surprised by how quickly they are expected to transition to the next station. You need to practice timing yourself as much as the content. You should practice the following:

• Time yourself reading the scenario and practicing what you will say at the start of your response during the mock station
• Practice the opening sentences of your responses until they are automatic.
• Rehearse examination techniques for clinical stations with a partner to perfect the flow and remove the thinking component
• Candidates should practice communicating with parents/caregivers about the examination process to help build rapport and enhance the overall examination experience

Adapting to Children’s Unpredictable Nature

In your MRCPCH Clinical examination you should always be expecting the unexpected. Remain calm, and be flexible to modify your behavior to suit the situation. This is super important in clearing the evaluation process of MRCPCH Clinical. Your patients will at most times be restless. You will often have to deal with distractible and uncooperative children. As a skilled paediatrician you should always be able to handle such cases.

When examining children and families, you must practice demonstrating a warm and caring attitude and applying your examination skills in a way that gently addresses the concerns of the children and their families. A Gentle and caring examination which explains the process to the family and demonstrates warmth and understanding will gain you more marks than a perfectly executed and technically correct examination which the family does not understand. It is important that you maintain a conversational tone during the examination and provide an explanation to the family of what you will be doing next.

Remember to engage in effective communication and maintain a professional attitude, because the examiners evaluate these aspects and give you credit, even if you do not arrive at the correct diagnosis.
